Purpose of the Job*
Provide an effective information and support service, based on a social prescribing model, to people referred by Primary Care staff or via self-referral.

Oxfordshire Mind Primary Care Wellbeing Workers can offer triage calls and up to six one-to-one sessions with clients who may be experiencing mental health issues or other social or lifestyle issues that are impacting on their wellbeing. Wellbeing Workers will work in a non-clinical person-centred way to encourage and enable service users to link in with existing support services, use the support available in their local community and develop tools to increase their ability to manage their own wellbeing.

This role is primarily for the Bicester Primary Care Network, however, you will also deliver wellbeing work for other Primary Care Networks on a flexible basis in order to support the wider capacity of the service. Where distance is a factor in covering out-of-area appointments, these can be delivered remotely via telephone.

**Responsibilities**
The key responsibility of the Primary Care Wellbeing Worker in this role is to provide an excellent person-centred service via triage calls and wellbeing sessions.
Triage
- Work directly from a daily triage list of telephone calls or eConsults from people who have concerns about their mental health or wellbeing.
- Work together with the client to agree the most appropriate next steps, e.g. signposting to another service, booking a wellbeing session (see below), or booking an appointment with a GP.

Wellbeing Sessions
- Provide initial assessment sessions and follow-up sessions as required.
- Work with individuals to identify their own hopes and goals, and support them to develop a personalised action plan to increase resilience and wellbeing.
- Review and assess progress of individuals, supporting them to complete their personalised action plans.
- Carry out follow up contact with individuals to evaluate outcomes using designated measures such as the Short Warwickshire-Edinburgh Mental Wellbeing Scale (SWEMWBS) and patient satisfaction questions.
- Appointments can be offered face-to-face or remotely, via telephone or video. This may vary depending on room availability at GP practices, service user requirements, or the ongoing situation with the Coronavirus pandemic.

Other Responsibilities
- Collect and accurately record service user data onto an online database.
- Keep up to date with appropriate information about the wider Oxfordshire Mind Wellbeing Service - and other local, regional and national services - to ensure service users can be offered appropriate information and signposting.
- Provide advice and guidance to GP practice staff about community and voluntary sector services.
- Forge and maintain excellent relationships with Primary Care staff and ensure the service is well understood.
- Promote excellent partnership working within GP practices including with all primary care staff, other practice link workers and other key stakeholders.
- Liaise with practice staff to ensure the service remains accessible to all individuals and groups e.g. younger and older adults, women with children, black and minority ethnic communities, veterans and groups who may have difficulty in accessing services.
