**Job summary**

Aldershot Health PCN are looking for a Cancer Care Co-ordinator and Cancer Champion to join our growing team of Administrative Care Coordinators within Aldershot Health Primary Care Network

Your role will be providing patients with a personal link while waiting for a referral or those patients newly Diagnosed with Cancer.

You will be provided with the system training required to assist practices with the administrative support they need to ensure patients who may have a cancer diagnosis are safety netted, using the EMIS Web clinical system reporting functions

Your role will include supporting the practices to promote Early Cancer Diagnosis and Cancer Prevention.

In so doing your role will be pivotal in increasing screening uptake to ensure that as many of our patients as possible who may have a cancer diagnosis are found and diagnosed early in the disease, giving them the best opportunity of treatment and recovery.

Supported by the PCN Cancer Clinical Lead and PCN Manager, you will work closely with the patient and their clinician and other healthcare professional, co-ordinating patients’ healthcare and directing them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

Your Cancer Champion work will aim to reduce healthcare inequalities in the Aldershot Health PCN area.

You will work with a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, Clinical Pharmacists, and pharmacy technicians.

You will be instrumental in supporting the patients and the wider team with the delivery of the Early Cancer Diagnosis Direct Enhanced Service.

**You should**

Be self-motivated and capable of integrating into teams within the four General Practices, whilst also being able to work on your own.

Have excellent organisational, interpersonal and communication skills.

Be willing and a fast learner to undertake the EMIS Web system training required to support you in your role.

Be willing to work both remotely as well as at the practices.

The role will be working for the 4 practices of Aldershot (Aldershot Health Primary Care network) under contract with Salus Medical Services

Salus Medical Services Limited is a federation of 20 GP practices across Northeast Hampshire and Farnham under Frimley CCG. We are dedicated to enhancing patient care and improving the health of local people. We are exploring innovative and exciting new ways of working to support general practice and to develop working at scale.
