Community Mental Health Practitioner

Hywel Dda University Health Board are currently looking to recruit a suitably qualified and motivated individual who has an interest in working with older adults to join the North Carmarthenshire Older Adults Community Mental Health Team based at Glangwili General Hospital. The post is permanent, part time.
Essential to the post is the ability to work in collaboration with the multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface.
The individual will be expected to respond to crisis situations, exercising clinical judgements and assessing complicated events and situations within client's own home, in other care settings and as part of their duty rota involvement.
The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload. The post holder will also be expected to undertake the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder will be responsible for supervision, PADR’s and caseload management support of the Health Care Support Workers within the CMHT. The post holder would be expected to deputise for the Band 7's in their very short-term absences.

**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest.
**Five community hospitals**: Amman Valley Hospital and Llandovery Hospital in Carmarthenshire, Tregaron Hospital in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.
Two integrated care centres (Aberaeron and Cardigan, Ceredigion).
**Community facilities, including**:
48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
