- ASYE Completed
- Extensive Post Qualifying Social Worker experience
- Knowledge of relevant social work legislation such as the Care Act and Capacity Act
- Knowledge and some experience in Hospital/Adults Safeguarding teams, risk assessments and risk management.
- Social Work England registered

NonStop's client is a local authority based in the Merseyside area is offering a unique opportunity for an experienced Social Worker to become part of the Hospital Integrated Discharge Team. The Council's expectations are high and you will need a strongcommitment to providing first class person centred care to residents over 18 and their carers.

You will be a committed team player, dedicated to Multi agency team working within the hospital setting and be able to manage and organise your own case load.

The Social Worker role involves discharge planning, working within the time frames of the Care Act. You will need experience in completing assessments and planning to facilitate safe and timely discharges. You will be passionate about working with a rangeof service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties), whilst challenging discrimination and promoting independence.
