Cwm Taf Morgannwg University Health Board is committed to improving the Health and Wellbeing of the population and reducing inequalities. The Health Board is leading the way in implementing a Population Health Management approach to improve population health outcomes. The local public health team plays a vital role in ensuring the Health Board and partners use data and evidence to inform population health-focussed decision making, planning and delivery.
In committing to implement this approach, a number of roles have been created. This includes senior analytical capacity alongside Senior Public Health Practitioner roles to support the Health Board to integrate Population Health Management into services.
