We're looking for a Rehabilitation assistant to join our expanding Rheumatology Team.

The Rheumatology team provides a service to the community living in the Harrogate & Rural locality of the Harrogate and District NHS Foundation Trust. Practitioners work within multi-disciplinary teams to ensure that individual patient needs are met.

You will help us aim to improve patient satisfaction and enhance self
- management with condition appropriate supporting information, advice and education.

Work on promotion of health and well being to empower patients to better manage their own health. As well as, signposting to additional resources and organisations to support self-efficacy.
- To carry an agreed clinical caseload as part of the Rheumatology therapy team, providing defined assessment and therapeutic input, either on an individual or class basis.
- To work independently at times to run gym, hydrotherapy and general exercise classes in the hospital and community setting, as required
- Advise patients who may have diverse and complex presentations
- To support a care plan following the initial assessment of a patient by a Specialist Rheumatology Therapist (Physiotherapist and Occupational Therapist). This may include the gathering of history and physical examination in order to plan and deliver an effective therapy intervention.
- To be able to devise, teach and supervise a patient performing an exercise programme and to progress in a safe and timely manner.
- To issue equipment, following the assessment by a Specialist Therapist, including walking aids, wrist and hand splints and supports and small adaptive aids.
- To be able complete a range of outcome measures, including participation in the collection of data and information and set patient-centred SMART goals.
- To assist with the supervision and support of therapy students within the department.
- To actively participate in continued professional development of your own learning
- To work flexibly as a member of the therapy department responding to the relevant needs of the service.
- To assist with income generation initiatives and promote efficient use of HDFT resources
