Here at** Derian House Children’s Hospice, **we have an exciting opportunity available for a **Young Adult Services Lead **to join our team based in **Chorley, Lancashire. **You will join us on a **full-time, 24-month fixed-term contract **working 37.5 hours per week and in return, you will receive a competitive salary of **£**37,657 per annum.**

**Derian House Children’s Hospice** is proud to have been providing high-quality care to children and young people with life-limiting and life-threatening conditions for the last 29 years.

LifeNoW are working in partnership with Derian House to provide this exciting opportunity. As our **Young Adult Services Lead**, you will work alongside the team at Derian House Children’s Hospice to facilitate and deliver holistic nursing care, including end-of-life care to young adults aged 21-28 with life-limiting and/or life-threatening conditions. You will be instrumental in developing the young adult services including links to adult palliative care providers and other appropriate services.

**About the** **Young Adult Services Lead role**:
The main purpose of the **Young Adult Services Lead** role is to work with young adults with a life-limiting condition to offer guidance and support for transition from children’s hospice services, to raise aspirations, help remove barriers and also to develop transition pathways and services which will promote autonomy and independence.

**What we’re looking for in our Young Adult Services Lead**:

- NMC registration
- A minimum of 3 years post registration experience
- Service Development experience
