Field Support Supervisor

Wolf Healthcare are recruiting for one of their most prestigious clients in the St Annes/Blackpool area. Looking for a Team Leader / Field Support Supervisor on a full-time permanent basis paying £27,690 per annum based on a 37.5 hour permanent contract to cover 2 supported living services assisting individuals with complex needs, self-harm and challenging behaviour.

Service one: 4 flats in St Annes (based here)

Service two: 6 bedded supported living service in Blackpool

Working for an established registered care provider with over 40 years’ experience who specialise in assisting those with complex needs, challenging behaviour, autism, and physical disabilities. This company has a great track record with staff retention and internal progression.

Shifts vary from 7am-10pm and then on occasions, you would be expected to do spot checks on Waking Nights 10pm-7am.

**Responsibilities of a** **Team Leader / Field Support Supervisor**:

- Creating and reviewing person centred care plans
- Ensuring high quality of support is being delivered to all service users.
- Assessing new service users and the support required and develop person centred care plans.
- Supporting people to take an active role is all aspects of daily living and promote independence.
- Recording and completing all. Appropriate documentation to ensure individuals records, observations and providing to direct Line Manager
- Completing medication audits, including administering, ordering, and returning of medication
- Oversee and maintain compliance for staff personnel files.
- Conduct supervisions, appraisals, and team meetings to ensure affective mentorship and communication across the team.
- Attend and participate in multi-disciplinary meetings with and for people supported.
- Ensure high quality care is being delivered throughout the services through observations and audits.
- Participate in on call service in a rota basis.

**Person specification for** **Team Leader / Field Support Supervisor**:

- Recent experience working in a Senior Team Leader capacity.
- Currently hold or happy to work towards Diploma level 3.
- Fully flexible to work a variety of shifts.
- Understanding of CQC legislation and requirements.
- Strong organisation skills.
- Experience writing and updating purposeful care plans.
- Good communication and soft skills.
