Job Summary

Reablement Team/Home First Services

The Team provides a responsive, multi professional rehabilitation services across the whole borough of Doncaster. The service is provided 24 hours a day, 7 days a week, the service aims to offer an alternative to hospital admission, facilitate earlier hospital discharges and promote optimum independence for patients within their usual place of residence. If you can demonstrate skills, experience and knowledge base relating to client focused, multi-disciplinary team working then we would like to hear from you.

Main duties of the job
- A central element of your role would be to provide support and assistance with personal care needs (e.g.bathing and toileting)
- Meal preparation
- Supporting with activities of daily living to enable clients to be as independent as possible and to remain in their own home.
- Vital observations
- Follow a plan of care as prescribed by the relevant professional
- Report back to qualified staff all findings and evaluation
