We currently have several opportunities for experienced Social Workers across different teams within our North Essex area.

**Neighbourhood Team**

In our aim to align more closely with our Health partners, and partners within the voluntary sector, Adult Social Care is moving towards supporting a neighbourhood model. Working in this way, teams are aligned to specific post code areas managing a triageprocess, completing assessments, Reviews and Safeguarding enquiries under the care act 2014 supporting an all-age approach. In North there are 5 Neighbourhood teams covering either Colchester or Tendring.

ASC Neighbourhood teams undertake incoming social work for all age specialisms. All our professionals use a strength-based approach when working with an adult to maintain their independence in a person-centred way to achieve their goals and develop resilienceand networks to maintain their wellbeing.

**Older Adults Mental Health**

The Older Adult Mental Health Team North is based in Colchester, covering both Colchester and Tendring, and the team has an exciting opportunity for a social worker to join the team. The role will involve working with adults aged 65 years and over diagnosedwith a functional or organic mental illness such as dementia; and their families both in the community and developing discharge pathways from inpatient wards in accordance with the Mental Health Act 1983 and Care Act 2014 principles.

This is a dynamic role promoting innovative person centred and partnership working with adults, their informal carers and in collaboration with other statutory, and non-statutory services to support and promote their mental health, well-being and outcomes.An essential part of the role is to challenge stigma, discrimination and consider least restrictive approaches for adults and their situations through holistic assessments and considering positive risk management to promote outcomes and recovery.

**PSI Team for Younger adults**

The North Essex Physical and Sensory Impairments Team cover the Colchester and Tendring Districts, with team members having the option of being based in either district whilst covering the whole operational area. We support people from eighteen years oldwith a wide range of conditions and difficulties including (but not exclusively): Acquired Brain Injury, Multiple Sclerosis, Stroke (CVA), Huntingdon's Disease and Fibromyalgia. The team is made up of social workers, a specialist sensory social worker, OccupationalTherapists and Community Support Workers.

**Learning Disability & Autism**

In North we have a Learning Disability and Autism Team. We are steering change within our local community by being part of improving outcomes for those living with a Learning Disability and/or autism in the North of Essex.

Our key aim current is to support a positive life course for those we work with, this includes the social care journey. As a team we have been continuing to develop a person-centred approach to practice, to support adults with achieving their goals and ambitions.While also supporting them to meet their assessed needs. We place a key emphasis on improving health outcomes, supporting employment and supporting adults with gaining independence through accommodation. This is where the exciting opportunity arises, we arelooking for social workers to join the team to become part of that journey, if you have a passion for supporting adults to achieve their full potential and want to gain further experience in improving outcomes and being part of this change then this opportunitywill be for you. You will have the chance to support adults from the very start of their journey, to reviewing their goals and ambitions further into their social care journey ensuring that their assessed goals and outcomes to continue to promote independenceand opportunities.

**Discharge to Assess Teams**

We are looking for applicants with the ability to demonstrate excellent practice. Proven track record of working closely with the community is essential. We'll give you the tools and resources you need to enable vulnerable adults to live healthily and independently.And, as you continue to grow your expertise, you'll play a key role in the development of our North Discharge to Assess Team.

Your support will be focussed on enabling adults to remain as independent and self-caring as possible in their own homes. However, some Adults may have been discharged to a temporary care home environment to aid their recovery and for a period of assessmentto be able to identify their short or long-term care and support needs. You will be working in collaboration with health care professionals and care providers using a multi-disciplinary approach to ensure the adults receive the right support at the right time.

**About You**

This role involves travelling around Essex. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.

As part of employment with Essex County Council (ECC) should your role require a DBS check you will be required to register with the DBS update service.
