Our services have a specific focus on individuals who present with high severity needs. We provide a broad range of specialist services. These services include specialist education schools and colleges for both day and residential learners, residential care homes and a therapeutic fostering services.
**Cambian Dilston College**
Dilston College provides further education to students with learning disabilities between the ages of 16-25. Our aim is to prepare the students for the next stage in their lives by developing their personal, social and practical abilities enabling them to develop employability skills and to live as independently as possible.
**Job Role**
To coordinate the transition of young people in and out of education and/or residential, working closely with stakeholders to ensure a smooth transition to Dilston College. The coordinator will support and deliver an intensive person-centred transition process on a cross college basis ensuring that young people are fully prepared and supported in achieving their destination goals.
You will be enthusiastic, committed and flexible. You should have experience of person-centred tools and approaches and have experience of working with young people with SEND in an educational context.
**Key Areas of Responsibility**:
- Deliver high quality transition, enrichment and personal and social development learning processes to support young people in achieving their destination goals.
- Deliver a provision that meets the Gatsby Benchmarks for careers guidance
- Facilitate and chair person-centred review meetings
- Deliver the wrap-around processes of support, information, advice and guidance, initial assessment and ongoing reviews as part of a cross college transition service.
- Contribute to assessment and monitoring processes in specific curriculum areas.
- Work with a caseload of young people and their families to provide them with impartial information, advice and guidance to support their future destination choices and assist their development of transition destinations.
- Work in close partnership with the range of other professionals supporting young people in transition e.g. Connexions, social workers.
- Work with the education team to develop a careers programme relevant to the young people and embedding this into the curriculum.
- Complete Education Health and Care Plan (EHCP) review paperwork, with input from relevant college staff.
- Ensure occupancy levels as per agreed KPIs
**Person Specification**
**Knowledge, Experience and Skills**
Knowledge of:
- Recent and relevant experience of the education and care service delivery requirements of people with a learning disability
- Working knowledge of the education and care operational contexts that influence service delivery
- Appreciation of the Ofsted, SEND CoP, and Care Quality Commission (CQC) and inspection regimes
- Teaching / care experience with learners with learning difficulties and/or disabilities
Experience of:
- Delivering and supporting transition within a person-centred curriculum in new and innovative ways
- Experience of facilitating person-centred reviews within education
- Delivering teaching activity within a 24 hours (extended) curriculum model
- Working with learners with challenging behaviour
- Delivering change in an educational and care environment
- Meeting delivery of targets and quality standards
