We have an excellent career opportunity for the role of **Care Coordinator** to join our amazing team here at **Aceso Home Care** to work at our head office in **Sandbach Cheshire.**
**Hours**: Full time role working 40 hours a week
**Pay**: We pay excellent salary of £28,000 per annum

Aceso Home Care is a Cheshire-based home care company, providing personal care and help with daily living across south and east Cheshire. Areas we cover include Nantwich, Crewe, Audlem, Middlewich, Winsford, Holmes Chapel, Sandbach, and Alsager.

Our recruitment process starts with background checks and an in-depth interview. **Key responsibilities**:
- Schedule and coordinate care services for our clients.
- Liaise between clients, caregivers, and family members.
- Support caregivers and ensure excellent care standards.
- Sharing on call with our office team.
- To be included in the care rosters
- Advantage of knowing Careplanner / Nourish system and Rostering experience would be an advantage
