An exciting opportunity has arisen for a highly motivated individual to join Norfolk Community Health and Care as Band 4 Discharge Co-Ordinators supporting 28 Intermediate Care Beds across 3 Care homes in Norwich. You will work with the multidisciplinary team liaising with family, patients and carers to plan and facilitate timely admissions and discharges from the care homes.

These post is substantive for 37.5 hours per week over 5 days, Monday
- Friday, no weekends or bank holidays.

**Duties include**: Making onward referrals to appropriate services. Liaising with patients and family. Forward thinking about discharging a patient from when they are admitted. Working within a multi-disciplinary team including Nurses, Doctors, Therapists, Social workers. Problem solving and leading weekly meetings. You will manage the admissions and discharges from the care homes and ensure accurate record keeping.

This is an exciting opportunity for any individual who likes to be proactive and work as part of a team.

On a daily basis you will either be at the Norfolk and Norwich University Hospital or located at one of the care homes to support admissions and discharges. You will lead weekly multi disciplinary meetings for all 3 care homes and take forward any actions identified. This may include liaising with family/other professionals, completing referral forms, speaking to patients, chasing up referrals, ordering transport.

You will need to be able to prioritise work and work unsupervised, as well as being part of the team. You will need good communication skills and be organised.
