Wirral Community Mental Health Service consists of two age inclusive teams covering defined areas within the Wirral Peninsular, one covering Wallasey, West and Southwest Wirral areas and one Birkenhead and surrounding areas.
The age inclusive team model will continue to recognise that patient’s needs may change as they get older and where they require the best skills to meet their individual needs.
The new model of care will address health and care needs and assist in the delivery of multiagency input. It will also enhance early detection and specialist management of mental health problems using the expertise of the multidisciplinary team whilst maintaining specialist input. This seamless provision will ensure that the specialty of delivering services to all service users is based on need and offer a holistic, person-centered, enabling service which addresses mental as well as physical health needs.
Birkenhead Adult Mental Health Service are looking to recruit an experienced Community Mental Health Practitioner who is an enthusiastic, self-motivated team player, and who is able to support and be actively involved in the further development of integrated working practice within the team.
The post holder will ideally have 1-year minimum post registration experience in working within the Mental Health Field and will be expected to work using a high level of expertise within the multidisciplinary team. The post holder must be able to work autonomously, have good communication skills, an ability to work in partnership with service users and carers and be able to work within a multi-disciplinary team setting. You will be expected to work within a Keyworker role and manage a defined caseload of clients with severe and enduring mental illness. In addition, the post holder will support the team in the future transformation of Community Mental Health Services.
This position requires travel across the service so access to a car for work and a full driving license are essential for the post.
