**We are looking to recruit a Care Coordinator to join our Home Care Office Team.**

We are looking to recruit someone who has excellent communication skills, great organisation skills, works well under pressure and can easily work both individually and as part of a team.

**As a Care Coordinator you will** ensure all customer visits are allocated and help with the recruitment of suitable Care Assistants whilst following appropriate office procedures.

**It is essential to have previous experience in Home Care and would be desirable if you have at least 1 year experience as a Care Coordinator.**

**Care Coordinator Principal Responsibilities**:

- Permanent allocation of Care Assistants to all customer visits and ensuring all visits always covered
- Ability to problem solve and manage challenging circumstances
- Answering all incoming telephone enquiries
- Processing new customer referrals
- Preparing appropriate documentation for Field Care Supervisors
- Administration of all Individual Care and Support Agreements
- Maintaining customer and Care & Support Worker office files and ensuring that they are always up to date
- Maintaining computer system up to date at all times
- Ensuring all customer and Care & Support Worker documentation and electronic records are kept up to date at all times.
- Ensuring all other records are kept up to date
- To work closely with the Field Care Supervisors
- To ensure compliance with all office systems and procedures
- Undertaking of supervisions for Care and Support Workers and monitoring their workloads
- To take part in the On-Call rota
- Previous use of Staff Plan or Similar would be desirable.
