Specialist Practitioner

We are looking for highly motivated and enthusiastic individuals who can work alongside colleagues, build a rapport with young people and families and support delivery of planned interventions.
The team will provide a package of enhanced mealtime support and intensive work with young people and families, usually within their own home, to prevent or reduce hospital admissions.
Alternatively, if a full admission is deemed clinically necessary, home leave and discharge may be facilitated sooner by the involvement of the EDIT Team.
The service will be provided 8am to 8pm, seven days a week therefore the successful post holders will be required to work flexibly, including evenings and weekends.
The service covers the large geographical area of Hull and the East Riding of Yorkshire so the ability to commute within this area is essential.
The main elements of the clinical work will include using an evidence-based model and care pathway to support the planning and delivery of a package of enhanced mealtime support and intensive work with young people and families, usually within their own home, to prevent or reduce hospital admissions.
The post holder will provide clinical support and guidance to less experienced members of the team whilst receiving a high level of training and supervision themselves.
