Eden Community Rehabilitation Team are seeking an experienced assistant Practitioner or someone looking to develop as a trainee AP within the ICC Community Team.
Working with a variety of conditions you will promote independence and management of Long term conditions. There is an opportunity to develop skills in both group work and individual interventions, supported by the experience Occupational therapist, Physiotherapist and Assistant practitioner. As part of the wider Community Rehabilitation team you may also be required to work throughout the wider team and ICC as service demands
The trainee assistant practitioner will be supported through the University Course attendance which also requires a commitment to home study out of work hours, Band 3 post until qualification.
To carry out independent simple assessment and patient -centred interventions under the guidance of the Occupational therapist/Physiotherapist.
Organise a delegated caseload of patients, identified by an allied health professional (physio or OT) as being appropriate for the intervention of an assistant practitioner.
They would also be expected to both take the lead and support in the delivery of various groups including falls. The workload requires the post holder to frequently work without direct supervision but would be under the guidance of professional staff.
Carry out visits in the community as required and deliver Home interventions which may include equipment and minor adaptations
They would also be expected to liaise closely with the multi-disciplinary team regarding treatment progression and ongoing care but have autonomy in progression of treatment within set protocols and also the ability to discharge.
