**Rate: Band 6, £30-£52 depending on experience.**

**6 month contract with view to extend**

**Overview of the role**:
The post holder will act as an ASD/LD Keyworker for children and young people who have complex needs across Northwest. The current focus of the project is to prioritise support to CYP up to the age of 25 who are placed in, or at risk of admission to, specialist mental health inpatient settings

To work with a small caseload of children and young people and their families to support them to build resilience and navigate and challenge the local system to facilitate person-centred care and timely support and interventions

**Responsibilities**:

- To troubleshoot on behalf of the child, young person and their family where required
- To act as the main point of contact for CYP and their family and strengthen links with relevant professionals.
- To attend and contribute to community and inpatient Care (Education) and Treatment Reviews
- To maintain clear, accurate records of work with each young person in line with local procedures on record keeping and data systems
- To take a proactive approach in engaging and liaising with stakeholders (including service users) that supports the provision of person-centred care

**Requirements**:

- Professional registration ideally within CAMHS remit such as: Social Worker, Nurse, teacher, therapist or significant experience working with children and young people with additional needs.
- Enhanced DBS check, children and adults

Contract length: 6 months

Pay: £30.00-£52.00 per hour

Expected hours: 37.5 per week
