**About the job**

You will sit within the practice team assisting in the implementation of best-practice care pathways, protocols, and therapeutics at Nourish. Used by care professionals, clinicians, friends, family and individuals these form a baseline for co-ordinating highly personalised care programmes. In combination with the Nourish app empowering care providers to deploy digitally enhanced care services that improve outcomes and daily lives for those they support.

You will work closely with the product team whilst reporting directly to the Clinical Safely Officer. This role will require a high level of collaboration with all Nourish teams, the Nourish user community, and those driving health and social care policy and practice.

**About you**

You will likely have a background in health and social care from a quality, operational or direct caring role. Preferably with experience in supporting individuals within community home care settings.

You will have a passion for working in the health and social care sector and a desire to improve outcomes through digital solutions with a willingness to keep up to date on current care guidelines, standards and regulations and how these are implemented in health and social care systems.

You will require Strong communication skills, both verbal and written, to effectively collaborate with internal and external stakeholders.

Be flexible and responsive to evolving needs and priorities.

Familiarity with the use of digital systems in a care environment, demonstrating an ability to use technology for improved care delivery. Empathy and understanding for diverse care types and sectors, to ensure inclusivity in pathway development.

**Key responsibilities**

Proactively seek customer feedback and identify pathways required for development to better support the users whilst adhering to current best practise guidelines and regulatory standards.

Support teams in software development priorities with a clinical mindset and knowledge whilst ensuring compliance with updated guidelines and best practises in care.

Assist in Identifying relevant pathways relating to specific care settings, episodes to include:

- Positive Behaviour Support
- Falls
- Nutrition
- Supported living
- Incident management
- Dementia
- Autism

Liaise with SMEs and other external professionals to support in the review and ongoing development of digital assets on the platform.

Regularly review existing and “in development” digital assets and programmes and escalate any concerns or issues to the clinical safety officer.

Support in the risk management of assets and programs delivered through the Nourish platform and monitor and address potential risks from a clinical POV.

Facilitate communication between the product team and other departments such as customer support, sales, training etc.

Ensure open communication channels regarding upcoming releases, metric measurements, and risk management.

Assist the clinical safety officer and product team in identifying the outcomes of programs being released on the platform.

Contribute to the evaluation and analysis of data to drive continuous improvement.
