**Care Home Impartial Assessor for West Hertfordshire Trust**

Based at Watford General Hospital but would be expected to drive to other hospitals including St Albans, Hemel Hempstead and Luton and Dunstable on occasion.

**What does the role of an Impartial Assessor involve?**

As a care home Impartial assessor, you will carry out initial assessments to facilitate discharge services at NHS hospitals by providing a detailed comprehensive individual assessment.

You will engage across hospital teams, care homes and social care to build relationships and trust. The individuals you will be assessing are care home residents, and your assessment will support the provision of high-quality care and assist in timely andsafe discharges.

You will need to gain and maintain a comprehensive knowledge of available local community resources and pro-actively liaise with the integrated team of health and social care workers, as well as other external health professionals, to maximise the supportavailable to the individual.

**Day to day duties will involve**:
- Support improvement within the hospital discharge team to improve and facilitate timely and safe discharges from hospital to care homes, improving the individual's experience by assessing, planning and implementing care in partnership with Care Home providers
- Liaise with the hospital wards to ensure a smooth discharge for the individual and care home and facilitate discharges where issues have arisen which could compromise the quality or timeliness of discharge from the hospital, working with all relevant staffacross organisational boundaries with a problem-solving approach.
- Assist and Liaise with care homes around the current discharge situations for the individual.
- Act as a point of contact when residents are admitted to hospital from care settings to monitor progress
- Where required provide data and reports on the service, including assessments, information dissemination, discharges to inform delivery of the service and where service improvements could be made
- Liaise with families when necessary to keep them informed and to support a safe and timely discharge for the resident.
- Where required, visiting care homes to create and continue relationships with managers in order to expand and develop the service and work to find solutions to any perceived barriers.
- Follow care home residents being admitted through their journey from front door to discharge to can support early discharge planning
- Identify themes why people are being admitted to Watford and then what training can be offered e.g falls and enablement, UTIs.
- Other potential admission and discharge projects for Care providers
**Am I the right person for the role?**
- Should have been involved in care home management as a unit manager, deputy manager or manager.
- Should have an understanding of the workings within care homes and hospitals and
- Experience or understanding of working within a hospital.
- Excellent communication skills both written and verbal
- Has passion and enthusiasm to raise the standard of quality of care for patients
- Has the ability to build rapport and negotiate with multiple different organisations
- Willing to learn new skills and to develop current knowledge
- A good understanding of IT is essential.
- An understanding of care act eligibility preferable but not essential.
