**Care Coordinator (Complex Care)**

**Heywood, Greater Manchester**

**£23 - £27k Basic + Oncall £3,960 per annum (based on completing oncall every 6 weeks approx.)**

The service we operate is specialist and Nurse Led, we provide highly trained care teams to work closely with clients in the community. We rely on Healthcare Assistants, Support Workers and Registered Nurses to support our service users within their own homes. Behind the scenes in the North West operation, we have a team of staff who work to co-ordinate and manage all aspects of our service.

**All applicants will be required to hold a full UK driving License**

- Previous care or care coordinator experience

**Main responsibilities as a Care Coordinator**:

- Building relationships with commissioners, professionals, clients and family members
- Rota Management for your packages of care
- Responding efficiently to day to day changes in your care packages
- Providing excellent customer service
