**OUTREACH SUPPORT WORKER - COMMUNITY BASED**

**THE ROLE**:
As an outreach support worker, you will be supporting our customers to live a fulfilled and meaningful life, that is positive, full of choice | Dignity and respect that allows a person to develop and grow.

Due to the Nature of the Role each person’s level of support will be unique and individually tailored to their needs so flexibility is key.

As part of this exciting role, you will support our customers in all aspects of their life such as supporting with Social and leisurely activities | supporting with medical appointments | supporting with daily living skills such as shopping | preparing meals | maintaining their home environment | managing finances | providing emotional support and a friendly face that they can rely on and trust.

**THE IDEAL PERSON**:
We are looking for people who have experience of working with adults with additional needs such as learning Disabilities | Autism | Mental Health | Behaviours that challenge

You will be calm and pragmatic, patient, understanding of a person’s needs | flexible in your approach | an excellent communicator| Empathetic | Strong Resilience | Excellent sense of humour and above all caring and compassionate.

**WORKING PATTERN**:
Due to the nature of the role, flexibility is key for this role and are formulated in line with the needs of the customer we are supporting, this could include short sessions | long days | Evenings | weekends

**Job Types**: Full-time, Part-time, Permanent, Zero hours contract
Part-time hours: 18 - 36 per week

**Salary**: £10.00-£10.30 per hour
