**Job summary**
A Young Persons Social Prescribing Link Worker helps to support and empower young people to take control of their health and wellbeing by giving them time to focus on what matters to me. A Young Persons Social Prescribing Link Worker uses a holistic approach to engage with young people aged up to 25 years old to provide advice, signpost and refer to positive activities and support within the St Austell area to improve physical and emotional health and wellbeing.
The post holder will respond to the unmet needs of young people in the St Austell area via the implementation of a children and young persons social prescribing model.
**This role is for 17.5 hours per week and will involve Saturday and / or evening working on a regular basis.**

**Main duties of the job**
The Link Worker will connect children and young people into a broad range of services and interventions in their local area in a timely and effective way, led by the wants and needs of the young person themselves.
The Young Persons Social Prescribing Link Worker will work within a team of adult & Young Persons Social Prescribing Link Workers and will work closely with local multi-disciplinary teams, and wider community groups and organisations - supporting existing groups to be accessible and sustainable, and help build new community groups, whilst working collaboratively with local organisations.
Young Persons Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing young peoples active involvement with their local communities
**Overview of your organisation**
St Austell Healthcare has a strong multidisciplinary team comprising GPs, ANPs, Nurses, Paramedics and Pharmacists and an ethos for innovation and collaboration with health and social care partners.

We are constantly seeking new ways of working to enhance service provision and the patient experience, introducing new technologies and methods of working to meet the expectations of our patient group. We are ambitious and wish to provide the best service we can for the people of St Austell.
**MAIN DUTIES AND RESPONSIBILITIES**:
1. Working with direct supervision from a GP and the Head of Social Prescribing, take referrals from a wide range of agencies, including wider multi-disciplinary teams, school nurses, student welfare services, youth and community services, primary health services, mental health services, youth offending teams, allied health professionals, fire service, police, job centres, housing associations, and voluntary, community and social enterprise (VCSE) organisations (list not exhaustive).
2. Provide personalised support to young people, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team. Develop trusting relationships by giving young people the time and focus on what matters to me. Take a holistic approach, based on the persons priorities and the wider determinants of health.
3. Co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting young people to community groups and statutory services.
4. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer young people back to other health professionals/agencies, when the persons needs are beyond the scope of the link worker role e.g., when there is a mental health need requiring a qualified mental health practitioner.
5. To promote the involvement of young people in planning, delivery and evaluation of services.
6. Draw on and increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups to receive social prescribing referrals.
7. Alongside other members of the PCN multi-disciplinary team, work collaboratively with all local partners to contribute towards supporting the local VCSE organisations and community groups helping them to become sustainable, and ensuring that community assets are nurtured by sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities.
