The role of a coordinator is to facilitate and assist the day-to-day operations of the specialty virtual wards community and urgent community response team, providing holistic clinical remote assessments such as coordinating care for patients, acting as a liaison between patients, families and health and social care professionals and taking on the management responsibilities necessary for successful service delivery.
The role entails working with the multi
- professional team contributing to providing and monitoring care and interventions in line with the agreed service pathways from clinician(s). The role will facilitate provision of health and social care needs of people to prevent unnecessary attendance and admission to hospital, thus providing holistic and person-centered care and support for people in a variety of settings.
The work base will be at New Alderley Building, at Macclesfield District General Hospital.
**Hours per week**: 37.5
The Virtual Ward / Urgent Crisis Response Coordinator will be part of the multi-disciplinary team (MDT) who are responsible for managing the care of people admitted to the specialist virtual wards. This will also involve coordinating the work of healthcare professionals involved in the community urgent crisis response.
To undertake a comprehensive holistic remote triage and clinical assessment of patients, including liaising with carers / relatives / other healthcare professional to identify risk, priority and urgency of need.
To facilitate safe early hospital discharge and prevent unnecessary admissions to hospital by facilitating an effective and successful interface between Community Urgent Community Response and Specialist Virtual Wards, ensuring smooth transition of patients with ongoing health and care needs to care community teams.
