The Role

As an Advanced Clinical Practitioner (Complex Care Nurse), you will:

  • Provide a range of routine and acute medical services, working autonomously and as part of the wider multidisciplinary team.
  • Deliver assessment, diagnosis, treatment, triage, and referral services within your scope of practice.
  • Work primarily in a patient-facing capacity, both in-practice and within the community, visiting housebound patients and care homes.
  • Focus on the management of patients with long-term conditions and provide general nursing interventions across the practice population.
  • Participate in the administrative and professional responsibilities of the practice, ensuring accurate and timely clinical documentation.
  • Collaborate closely with the Partners, Clinical Leads and Management Team to support delivery of our organisational priorities across our Tilbury practices.

This role is ideal for healthcare professionals with experience in primary care, community care, or out-of-hours services.

About You

You will be:

  • Flexible, hardworking and empathetic in your approach to patient care.
  • Confident in working both independently and as part of a multidisciplinary team.
  • Committed to continuous professional development and high clinical standards.

Essential qualifications and requirements:

  • Registered General Nurse (RGN) with a Master's degree in Advanced Clinical Practice (or equivalent MSc / NHSE Advanced Practice Digital Badge, or working towards completion).
  • Registered Independent Prescriber currently working in an advanced clinical role.
  • Strong ICT and clinical system skills.

  • Disclosure and Barring Service (DBS) Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a DBS check for any previous criminal convictions.
  • Immunisation: Successful candidates must provide a full immunisation history before commencing employment.
