Job overview

The Sutton Community Learning Disability (LD) Health Team comprises the LD Team, hosted by the London Borough of Sutton and located at the Civic Centre and the Sutton Inclusion Centre and the Dysphagia Team for Adults with LD (DTALD) hosted by Sutton Health and Care (SHC) and located at Green Wrythe Lane Health Clinic, Carshalton.

The team provides a service to adults with LD that are unable to access mainstream health services in the community. In Sutton, there are 42 LD care homes with 265 residents. To allow LD residents to stay in the care homes for as long as possible, the Growing Older with LD (GOLD) Project aims to facilitate regular reviews and screening and promote early intervention strategies to prevent hospital admissions, avoidable helath complications and promote integrated, person centred care.

Main duties of the job

The post holder will work as an integral part of the GOLD project, working with the LD nurses to increase the number of UCPs in the homes, increase the number of residents to have bowel cancer screening, annual health checks and understanding of their long term conditions.

SHC's vision is to deliver seamless excellent coordinated care that is person centred and of high quality for people at risk of an unrequired hospital admission and for those that required admission ensure they are supported promptly to get them home again as quickly as possible.

Detailed job description and main responsibilities

Clinical

  • Identify relevant patients and collect data for the Sutton Getting Older with Learning Disabilities (GOLD) project, including details such as age (60+), vaccination status and Health Care Passport information
  • Create referrals for the nursing team, encompassing Sutton GOLD and hospital discharge follow ups
  • Organise Universal Care Plans (UCP)/GOLD appointments, ensuring UCP information is sent in advance
  • Gather information from relevant parties and relay it to the administrative team for transcription and submission; upload UCP information
  • Manage the team's email correspondence
  • Add confirmed LD diagnoses to EMIS
  • Oversee the inpatient spreadsheet, providing information on confirmed LD diagnoses and open patient cases
  • Record meeting minutes for nursing and Community health team (CHT) meetings
  • Book nursing appointments
  • Under the supervision of the nurses or clinicians, carry out agreed follow up actions
  • Contribute to team meetings and service user reviews, ensuring their goals are continually reviewed and treatment plans adjusted accordingly
  • Effectively plan, prioritise and manage own workload
  • Promote protection of vulnerable adults, working according to policies and procedures
  • Maintain accurate records of all client care activities within Community Services and professional guidelines
  • Follow infection control policies, ensuring compliance with infection prevention and control practice and hand hygiene as a core component of basic infection control for self and within team
  • Ensure messages are transmitted within an appropriate timeframe and using methods consistent with their urgency
  • Support the team's administration as required.

Education, Development and Training

  • Comply with clinical governance activities
  • Identify and attend relevant or further training as appropriate to maintain skills and knowledge in the area of specialism (e.g. attendance at courses, meetings and specialist interest groups)
  • Ensure that adequate records of assessment, treatment and progress are maintained in accordance with the record keeping policy
  • Maintain patient confidentiality at all times
  • Review, update and develop dietary information and education materials for patients and carers
  • Support review, update and develop clinical policies, local protocols and procedures
  • Contribute to the development and updating of departmental evidence-based clinical standards, policies, procedures and patient informatin
  • Participate in research and audit within the specialist area
  • Participate in identified departmental working groups as agreed with the Service Manager
  • Attend and contribute to regular DTALD team meetings
  • Contribute towards maintaining and improving high standards of dietetic practice through the application of clinical governance initiatives, including participation in audit and research as appropriate
  • Participate in continuing professional development as identified through a performance development programme
  • Contribute to effective team working and to communicate with colleagues, patients and visitors in a polite and courteous manner at all times.

  • Provide training and education to qualified and unqualified staff in order to address and enhance the skills and knowledge of the workforce
  • Participate in clinical supervision, appraisal and in-service training as identified on personal development plan
  • Maintain clinical and professional competence by keeping up to date with nursing/allied health care and health care trends and developments by reading clinical/professional literature, research, study days and liaison with other professionals.

Service Development

  • When required, participate in the identification of service development needs
  • Support in the delivery of service key performance targets

  • Identify and make recommendations for service development and improvements
  • Be pioneering within the service ensuring continuous improvement of the quality and safety of services by taking action on feedback and seeking to develop skills and knowledge of self and others

  • Ensure contemporaneous records are maintained at all times to safeguard clients' confidentiality by being aware of legal and ethical responsibilities

  • Contribute to the development of learning disability knowledge
  • Support the implementation of evidence based practice.

  • Work in a team structure, liaising with colleagues, community teams and other members of the multi-professional team to ensure a high standard of timely patient care is delivered
