Assessment and Review Officer
Salary: £28.971| Permanent | Enhanced DBS | Full time
We're looking for a caring, proactive and skilled Assessment and Review Officer to join our Home First team. You'll play a key role in helping adults regain confidence and independence after illness, injury or a period of reduced ability — supporting them to achieve their personal goals and live fulfilling lives in their own homes.
*About The Role*
As part of our Reablement Service, you'll:
- Complete timely assessments, reviews and support plans that reflect each person's needs, goals and strengths.
- Coach and empower people to take an active role in their own recovery, using a reablement approach that builds confidence and independence.
- Assess and manage risk, ensuring safe, person-centred care.
- Work closely with colleagues, families, carers and partner agencies to deliver consistent and effective support.
- Support the deployment of care staff and contribute to the day-to-day delivery of a high-quality, CQC-compliant service.
- Teach and demonstrate the use of mobility aids and assistive technology.
- Maintain accurate, up-to-date electronic records and documentation.
This is a varied and rewarding role where no two days are the same. You'll make a real and lasting difference to people's lives every day.
*About You*
You'll be an experienced care professional with:
- Experience of assessing adult care needs, risk management and care planning.
- A background in social care, health care or community settings.
- Excellent communication, problem-solving and decision-making skills.
- The ability to work flexibly — independently and as part of a team — to meet service needs 7 days a week.
- A full driving licence and access to your own vehicle.
You'll also be self-motivated, compassionate, and confident in supporting vulnerable adults while maintaining the highest standards of safeguarding and professionalism.
*Qualifications:*
- Diploma/NVQ Level 3 in Health or Social Care (or willingness to work towards).
- Competency certificates in care, manual handling, and simple aids/adaptations (or willingness to obtain).
*What We Offer*
Join a team that invests in you. We offer a generous package of benefits designed to support your wellbeing, lifestyle, and career growth:
- Comprehensive induction and ongoing professional development.
- Support to gain recognised qualifications and competency certificates.
- A friendly, supportive and inclusive team culture.
- Opportunities to shape and develop the reablement service.
- Great Pension – 6% employer and 4% employee contributions with Aviva.
- Generous Annual Leave – 26 days + 8 bank holidays (rising to after 5 years).
- Flexible Holidays – Buy or sell up to 3 days leave (excl. term-time staff).
- Health & Wellbeing Support – Access to occupational health, short-term counselling, physiotherapy, and 24/7 confidential counselling.
- Healthcare Cash Plan – Claim back up to 100% of everyday healthcare costs (dentist, optician, physio, 24/7 GP consultations, and more).
- Life Assurance – Up to 3x your annual salary.
- Enhanced Pay – Maternity, paternity and sickness benefits.
- Lifestyle Perks – Discounts on groceries, high-street shopping, leisure, dining, airport parking, and vehicle maintenance (service, MOT, fuel cards, and insurance).
- Cycle to Work Scheme – Save on a new bike through salary sacrifice.
- Training & Development – Access to high-quality learning, including care-specific courses.
- Recommend a Friend Bonus – £500 for successful referrals.
- Annual Bonus – £250 contractual bonus plus additional performance bonuses.
- We're proud to offer more than just a job — we offer a supportive, flexible and rewarding workplace where you can grow and thrive.
