Complex Carer Support Worker

The Princess Royal Trust for Carers in Hampshire provides advice and support to family members, friends and neighbours who are caring for a loved one with a diagnosis, illness or who may be elderly and frail.
Due to continued expansion, an exciting opportunity has arisen to join this Hampshire based charity, as part of their Complex Carer Support Team (supporting the carer rather than the cared for). Covering Eastleigh and the Southern Parishes. The role will include sharing of knowledge and an ability to work with family members, unpaid carers and professionals. You must possess excellent communication, organisational and administrative skills as well as have recent knowledge of either health or social care.
Main duties will include:·
· To provide accessible advice, information, support and training to carers through adopting a strengths based, whole family and partnership approach.
· Respond to carers through a variety of approaches, including providing advice online, over the telephone, or meeting at a venue convenient to the carer.
· To provide carers with personalised information, advice and support on local and national services which will support their social, emotional and physical wellbeing and in using the strengths-based approach.
· To provide one to one support, when needed, at a time and place convenient to the carer.
· To promote and publicise the role of a carer, its importance and value in line with promoting and raising awareness of the Hampshire Carers Strategy.
· To ensure that all groups and individuals have access to information and support that is culturally appropriate.
· To ensure that barriers for those who find services difficult to access are minimised and identify and engage with 'seldom heard' or 'hidden carers'.
· To work in partnership with local organisation to ensure full-service integration.
· To develop and maintain a good local knowledge of relevant services, groups and named key contacts.
· To initiate, develop and maintain effective working relationships with other local professionals working in the field of adult health and care across the voluntary, statutory and private sectors to build awareness of opportunities for involvement and collaboration.
· To administrate and organise own work to ensure that it is accurate and meets quality targets, reasonable deadlines and reporting requirements.
· To collect qualitative and quantitative data on a weekly basis and forward to line manager.
· To participate in service evaluation, gaining feedback from service users. To advocate for carers rights and ensure they receive appropriate resources and assistance.
· To support carers and maintain a knowledge of a broad range of illness and conditions. A knowledge of financial support, money management, benefits and carer discount schemes.
· To deliver complex carer support and specific advice around continuing health care and carers with multiple issues.
Salary is £24,500
