Overview:
We practice from a new Health Centre in the pleasant, large village of Hope, located between Mold and Wrexham and 10 miles from Chester. Our patient list size is over 10000 and we dispense to 2000 patients, providing a quality dispensing service.
We are looking for an enthusiastic candidate to join our dynamic practice as a Pharmacist. This is a fantastic chance to work within a supportive multi-disciplinary team and play a vital role in delivering high-quality patient care through safe, effective medicines management.
You will work closely with GPs, nurses, and other healthcare professionals to support medication safety, chronic disease management, and prescribing initiatives, ensuring compliance with clinical guidelines and improving patient outcomes.
The job includes the following responsibilities:
· Completion of Medicine Management/Repeat Reauthorisation of chronic disease management.
· Hospital Discharge reconciliation and outpatient letters
· Medicine queries, for example, brand changes/formulation changes/out of stocks coming back from community pharmacies.
· Involvement in selected enhanced services
· Involvement in QI Projects
- Care home medication reviews
- Telephone-based medicines support
- Medicines information to practice staff and patients
- Medicines optimisation
- Repeat prescribing and promoting Electronic Repeat Dispensing
- Monitoring of High Risk Drug Alerts in accordance with local policies and procedures
- Medicines safety and quality improvement
- Implementation of local and national formulary guidelines

Person Specification
Experience
Essential
- An appreciation of the organisational nature of PCNs, GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients, doctors).
- Good IT skills. Microsoft Office Word and Excel.
- Able to obtain and analyse complex technical information.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and able to refer to seniors or GPs when appropriate.
- Able to work under pressure and to meet deadlines.
- Gain acceptance for recommendations and motivate the audience to comply with the recommendations and agreed course of action where there may be significant barriers.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
Desirable
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Ability to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Experience with EMIS Web clinical system.
Qualifications
Essential
- Independent Prescribing qualification
- Master's degree in pharmacy (MPharm) (or equivalent),
- Mandatory registration with General Pharmaceutical Council,
- CPPE Primary Care Pathway training course, or working towards / intent of completing
Desirable
- Membership of the Royal Pharmaceutical Society,
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience

- In date DBS check.
- Immunisation status.
- UK Work Permit (if required).
