Southport and Formby Primary Care Network (PCN) is a group of 14 GP Practices working together with other local organisations to improve the health and social care of our local population of around 129,000 people.

Primary Care Networks are designed to provide proactive, coordinated care to their local populations, in different ways to match different peoples needs, with a strong focus on prevention and personalised care. This means supporting patients to make informed decisions about their own health and care and connecting them to a range of statutory and voluntary services to ensure they can access the care they need first time.

Job descriptionJob responsibilities

The role will include the duties outlined below;

  • Work as part of a multi-disciplinary team in a patient-facing role

  • Working with GPs and patients to address medicine adherence.

  • Review medication and clinical processes for high-risk patients to ensure high-quality, evidence-based prescribing

  • Conduct structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy

  • Provide clinical medicines expertise to both patients and general practice team

  • Improve prescribing practices across the Primary Care Network

  • Interface with community and hospital colleagues to help resolve medication-related issues

  • Managing and prescribing for long-term conditions

  • Helping the practice deliver on the Local Quality Contract (LQC), Quality, Innovation, Productivity and Prevention (QIPP) and Quality Outcomes Framework (QOF) agenda and enhanced services.

  • To support the care homes within the PCN with structured medication reviews

  • To work with colleagues to establish and develop good medicines management systems across the care home sector

  • To assist the PCN Enhanced Health in Care Homes Team with care home related medication queries

  • Behave in a manner which is welcoming to and of any individual, is non-judgmental and respects their circumstances, feelings priorities and rights.

Person SpecificationExperienceEssential

  • An appreciation of the nature of GPs and general practices

Desirable

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing

SkillsEssential

  • Excellent interpersonal, influencing and negotiating skills
  • Excellent written and verbal communication skills
  • Produce timely and informative reports

Desirable

  • Good IT skills

Personal QualitiesEssential

  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Able to obtain and analyse complex technical information
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
  • Able to work under pressure and to meet deadlines
  • Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
  • Work effectively independently and as a team member
  • Demonstrates accountability for delivering professional expertise and direct service provision
  • Enhanced Disclosure Barring Service (DBS) Check

QualificationsEssential

  • Mandatory registration with General Pharmaceutical Council
  • Master's Degree in Pharmacy (MPharm) or equivalent
  • Independent prescriber or working towards/intent of gaining independent prescribing qualification

Desirable

  • Membership of the Royal Pharmaceutical Society
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
