You will work alongside the Children's Continuing Care Nurse to provide family-centred nursing care for children with complex needs in the community. Care will be delivered across a range of settings, including homes and schools within the Bedford and Luton area.

The role involves providing care and support for individuals with complex needs on a 24-hour basis, which includes both day and night duties as part of a rota system. Flexibility is essential to ensure continuous care and meet the needs of service users.

With the support of the children's community care nurse provide high quality family centred nursing care

for technology dependent children and children with complex needs in the community.

Care may be delivered in a range of settings appropriate to the children (e.g. school/home/respite unit) over the 24hr

period.

The post holder will be required to work in partnership with children and young people with complex needs, their families, and the multi-disciplinary team to enable the child to attain the optimum life experience.

Main Duties and Responsibilities

To recognise own strengths and weaknesses and ask for help when unsure – from the parents or children's continuing care nurse

To build professional relationships with families who may be stressed and finding it difficult to cope with a child who has very complex health needs

Communicate effectively and appropriately with children who have complex health needs and/or learning difficulties

To carry out nursing tasks as per daily care plan

To ensure a safe environment and the safe use of equipment and reporting any unsafe environment or unsafe use of equipment to the Team Leader

To identify changes in the child's condition assessing and referring appropriately to parents and/ or community nurse

In an emergency know what to do and who to contact

Some Line Management of Junior staff following training

Communication

Communication Ensure that all organisational-wide communication is accessed and adhered to accordingly via the and/or account given upon start of employment. This also applies to communication sent via internal or external mail, fax, or memo and LCS Intranet

People Management

Parents and families of children with very complex needs

Children's Community Nurses

Carers from other agencies e.g. social services, nursing agencies, schools

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken regarding the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes

The post holder must adhere to infection control policies and procedures

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in Clinical & Safeguarding audits as required

Provide patients and their families /carers with information on standards they should expect from the team

Participate in clinical supervision on a regular basis

The post holder is required to participate in relevant emergency preparedness process for their team.
