We are looking for a dedicated Community Mental Health Support Worker/Assistant Practitioner to join our Mental Health Integrated Network Team (MINT) . This is a key role focused on improving health, wellbeing, and care outcomes for individuals referred by their GPs, inpatient mental health units, crisis teams and single point of access
About the Role
In this rewarding role, you'll support individuals to:
Access local community-based services to aid their recovery
Engage meaningfully with their community
Develop personalised support plans based on their strengths, needs, and goals
You'll also:
Carry out assessments and co-create support plans
Maintain a live register of community resources
Work closely with GPs, social care teams, and voluntary sector partners
Respond appropriately to safeguarding or crisis concerns
Help shape a system-wide community asset map
Key Responsibilities
Manage a caseload and offer one-to-one support
Connect clients to relevant community services and activities
Work collaboratively within the wider MINT team and external partners
Monitor outcomes and maintain accurate records
Participate in service improvement and professional development activities
What We're Looking For
A strong communicator with a passion for mental health and community engagement
Experience (or a strong interest) in health, social care, or community support work
A proactive, solution-focused approach and a commitment to person-centred care
An understanding of safeguarding, confidentiality, and working in multi-agency environments
