Job Description Job Title: Community Advanced Clinical Practitioner (ACP) Prescriber Reports to: Geraldine Scholtz - Line Manager Location: Meridian Health Group - Community Team Job Summary The Community Advanced Clinical Practitioner (ACP) will provide expert, autonomous clinical care to patients in care homes and those who are housebound. The postholder will use advanced clinical skills, diagnostic expertise, and independent prescribing to assess, manage, and review patients with complex health needs. They will play a key role in supporting proactive, person-centred care, reducing avoidable hospital admissions, and improving quality of life for vulnerable populations. Key Responsibilities Undertake comprehensive clinical assessments of patients in care homes and at home, including physical, psychological, and social aspects of care.

Formulate differential diagnoses, request and interpret investigations, and implement evidence-based management plans. Prescribe and review medications independently in line with professional standards and local protocols. Provide proactive management of long-term conditions, frailty, palliative care, and end-of-life care in collaboration with the multidisciplinary team (MDT). Respond to urgent and unplanned care needs, making timely clinical decisions to prevent unnecessary hospital admissions.

Act as a clinical resource for care home staff, families, and carers, providing education and support to enhance patient care. Work in partnership with GPs, community nursing teams, pharmacists, therapists, and other MDT colleagues to deliver integrated care. Contribute to clinical governance through audit, quality improvement initiatives, and safe prescribing practices. Maintain accurate, timely clinical documentation in line with professional and organisational requirements.

Participate in supervision, professional development, and ongoing training to maintain and advance clinical competencies. Qualifications & Experience Essential: Registered healthcare professional (NMC / HCPC) with current registration. MSc (or equivalent) in Advanced Clinical Practice. Independent prescribing qualification (V300 or equivalent).

Significant post-registration experience working in a senior clinical role. Proven ability to manage patients with complex health and social needs in a community setting. Desirable: Experience working in care homes, community, or primary care. Knowledge of frailty assessment tools, advanced care planning, and end-of-life pathways.

Evidence of leadership in clinical practice, education, or service development. Skills & Competencies Advanced clinical assessment and diagnostic reasoning. Strong communication and interpersonal skills, with the ability to build trust and rapport with patients, families, and staff. Clinical decision-making under pressure and in unpredictable environments.
