Responsibilities:

  • Provide direct care and support to individuals with physical and intellectual disabilities
  • provide coordination and navigation of care and support across health and care services

    -Review service users' needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.
  • Maintain accurate documentation of care provided

Requirements:

- High school diploma or equivalent

- Previous experience in a caregiving role preferred

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and patient demeanor

- Valid driver's license and reliable transportation

- Ability to pass a background check
