Salary: Agenda for Change Band 7 £56,276 - £63,176 pa dependent on experience (includes HCA Supplement)

Hours: Full time - Approximately 150 hours over four weeks. Worked in 12 hours shifts 8am - 8pm, 7 days/week (inclusive of weekends and public holidays).

Length of Tenure: Permanent

We are looking for an experienced specialist palliative care nurse who can operate within a fast paced, dynamic and flexible specialist palliative care rapid response service.

This post is suited to a candidate with significant experience of caring for patients with life limiting illnesses within a specialist palliative care setting (or aligned field) and who can demonstrate expertise in holistic assessment, palliative symptom management and advanced communication.

You will be a key member of the rapid response team, working closely with the wider multidisciplinary team both in the hospice and externally, fostering strong links whilst working across Royal Trinity Hospice's community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.

Key responsibilities

Carry out targeted holistic assessment of patients' specialist palliative care needs and develop an initial care plan to address any urgent needs.

Communicate and liaise with the patient's usual care team including their GP, District Nurses, existing specialist palliative care professionals, out of hours services, reporting on the condition and needs of the patients and providing expert advice and support to address their urgent needs.

Establish excellent working relationships with internal and external peers and colleagues to foster positive and collaborative working.

Offer support, guidance, and education to less experienced members of the rapid response team, and other professional visitors to the team to encourage individual and team learning and development in specialist palliative care.

Skills and experience required

Registered Nurse with bachelor's degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.

Advanced assessment skills.

Advanced communication skills.

Enthusiasm and a flexible attitude to working.

Ability to work autonomously and as part of a team.

The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is essential).
