Job Title: Advanced Practitioner – Transitions Team
Location: Nottinghamshire
Rate: £43.53 per hour
We are seeking an experienced and dedicated Advanced Practitioner to join our Transitions Team based in Nottinghamshire.
Primary Responsibilities as an Advanced Practitioner:
- Provide high-quality support and guidance to young adults transitioning from children's services to adult services.
- Work collaboratively with multi-disciplinary teams to ensure a smooth transition process.
- Conduct assessments and develop personalized care plans.
- Monitor and review the effectiveness of care plans and make necessary adjustments.
What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK license
- Post-qualified experience
- Degree level or equivalent in Social Work
