Job Overview

We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the highest level of care and support throughout their healthcare journey. This position requires excellent organisational skills and the ability to work effectively in an office environment while liaising with various healthcare professionals, patients, and their families.

Duties

  • Coordinate patient care by scheduling appointments, managing referrals, and ensuring timely follow-up on treatment plans.
  • Serve as a point of contact for patients, addressing inquiries and providing information regarding services and resources.
  • Collaborate with healthcare providers to develop and implement individualised care plans tailored to each patient's needs.
  • Maintain accurate records of patient interactions, care plans, and progress notes in compliance with relevant regulations.
  • Assist in the management of patient data and documentation to ensure confidentiality and security.
  • Facilitate communication between patients, families, and healthcare teams to promote continuity of care.
  • Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.

Qualifications

  • Previous experience in a healthcare setting or similar role is preferred.
  • Strong organisational skills with the ability to manage multiple tasks efficiently.
  • Excellent communication skills, both verbal and written, with a focus on empathy and active listening.
  • Proficient in using office software applications for documentation and scheduling purposes.
  • Ability to work collaboratively within a team while also being self-motivated.
  • A commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare.
