Role Purpose

Are you passionate about supporting people at a critical point in their lives? Join our hospital-based social work team, where you'll help BCP residents following a hospital admission. We are recruiting for a full-time Social Work position. You'll be based primarily at Poole and Bournemouth Hospital sites, working closely with health partners and community teams to deliver person-centred care.

You'll be part of a highly motivated and supportive hospital social work team, working across health and social care services. The team includes Qualified Social Workers, newly qualified ASYE Social Workers, Apprentice/Trainee or Student Social Workers, and experienced unqualified Social Care staff. We celebrate and share good practice and will give you the training and development you need to excel. We are committed to supporting your continual professional development. Main Responsibilities

  • Work collaboratively within multi-disciplinary teams (Occupational Therapists, Physiotherapists, Ward based medical professionals, Homeless Intervention, Housing, Reablement Teams, Dorset HealthCare, All Age Continuing Care).
  • Undertake strengths-based, person-centred assessments and discharge planning.
  • Advocate for individuals and carers, ensuring their voices are heard and their needs are met.
  • Encourage independence, helping people build confidence and access tailored support.
  • Manage positive risk and make evidence-based decisions in a fast-paced hospital environment.
  • Develop strong local knowledge and work in partnership with providers and the voluntary sector.
  • Provide creative, out-of-the-box solutions to complex challenges.
  • Maintain high professional standards in line with statutory, legal, and policy frameworks

The Ideal Candidate

You will support the 'Discharge to Assess' model, working alongside different Reablement providers who have supported people to return home.

You will also support those adults who have been transferred from an acute hospital into a Discharge to Assess bedded option in the community for further assessment.

You will also support those who need more complex discharge planning from the acute hospital, for example, those who may need further mental capacity assessments or best interest decisions, safeguarding or court of protection.

  • You will hold a professional Social Worker qualification, i.e. Degree in Social Work, CSS CQSW, Dip SW – All Levels.
  • Experience ideally within hospitals or with adults/older people.
  • Innovative, creative, and solution-focused approach to supporting independence.
  • Passionate advocate for others, with excellent communication and partnership skills.
  • Willingness to travel across the BCP area and work flexibly.
  • Commitment to person-centred, strengths-based practice and continuous professional development.
