What's the Role?
We're recruiting Lead Workers. You'll:
- Build positive, trusting relationships with adults and carers, focusing on what matters most to them.
- Use strengths-based, person-centred approaches to help people achieve their personal outcomes.
- Lead and coordinate multi-agency support, working alongside health, housing, and community partners.
- Support people with multiple needs, helping them to live well and independently.
- For Senior Lead Workers: Supervise and develop colleagues and play a key role in shaping our service and culture.
