Job Summary

The Care Coordinator plays a vital role in ensuring that clients receive the highest quality of care and support. This position involves working closely with clients, families, and healthcare professionals to facilitate effective communication and coordination of services. The ideal candidate will possess strong organisational skills and a compassionate approach to client care.

Duties

  • Serve as the primary point of contact for clients and their families, providing information and support regarding care services.
  • Coordinate appointments, assessments, and follow-up visits to ensure continuity of care.
  • Maintain accurate and up-to-date client records in accordance with privacy regulations.
  • Collaborate with healthcare providers, social workers, and other professionals to develop comprehensive care plans tailored to individual client needs.
  • Monitor client progress and adjust care plans as necessary to achieve optimal outcomes.
  • Assist clients with navigating healthcare systems and accessing community resources.
  • Conduct regular check-ins with clients to evaluate satisfaction with services provided and address any concerns.
  • Organise office operations, including scheduling meetings, managing correspondence, and maintaining office supplies.

Requirements

  • Previous experience in a healthcare or social services setting is preferred but not essential.
  • Strong organisational skills with the ability to manage multiple tasks effectively.
  • Excellent communication skills, both verbal and written, with a focus on building rapport with clients.
  • Proficient in using office software applications for documentation and communication purposes.
  • A compassionate attitude towards clients and a commitment to providing high-quality care.
  • Ability to work independently as well as part of a team in a fast-paced environment.

Job Type: Fixed term contract

Contract length: 6 months

Pay: £14.00-£14.50 per hour

Expected hours: 37.5 per week
