In our quest to deliver patient-focused quality care we are looking to recruit a highly motivated, innovative and enthusiastic Neurodiversity Complex Case Lead to join our forward thinking, friendly and expanding Health & Justice team at HMP Send .
This post can also be offered as a band 6-7 developmental post, this will be a fixed term contract and it is expected that progression be achieved within 12–18 months, depending on individual readiness and development progress
By join ing our Prisons Health Care Team, you will not only look after the healthcare needs of our patients residing in secure environments , but who also can have a real impact on promoting health and well-being. Our healthcare services work to a 7-day week working model , to ensure our patient's healthcare is always managed and maintained to the highest of standards .
Our motto is "Caring NOT Judging" so by w orking in partnership with the criminal justice system and other agencies, we provide responsive, dependable and high-quality primary care and mental health services to those who may have offended and those at risk of offending in the communities we serve, to improve their health status and to encourage social inclusion.
- Lead in the identification, assessment, support of people with Neurodevelopmental Disorders, including Autism, Attention deficit Hyperactivity Disorder, Intellectual & Developmental Disabilities.
- Provide & support others to offer adapted psychosocial interventions for people whose communication, thinking style & coping strategies are impacted upon by the presence of a ND.
- Hold a complex caseload of people who are identified to have needs associated with ND & require reasonable adjustments to be made to their healthcare.
- · Keep up to date with research, policy & legislation relevant to ND.
- Offer expert consultancy to the Mental Health Team & contribute to person centred pathways & interventions within the MDT.
- Develop person centred Care Plans which identify strengths & needs in collaboration with individuals.
- Supported by your clinical lead to improve the outcomes of people with ND at HMP Send, delivering against the national specifications for primary & mental healthcare for people with ND.
- Deputise in the absence of line manager when required & play an active part in the Healthcare Management Team.
- Recognise that there is need for flexibility, working outside of normal working hours if required, including weekends.
- Maintain own skills & knowledge to a high standard & ensure that this is reflected within the team.

QUALITY OF CARE
STANDARD
Promote the health and well-being of patients and maintain a safe, caring, and therapeutic environment based on an individualised approach. Working with the Service Manager to provide an environment that promotes training and development among the workforce.
INDICATOR
To maintain a high standard of care to patients and ensure reasonable adjustments made to meet the needs of people with ADHD, ASD and IDD, including communication disorders.
To undertake work needed to support research and develop care pathways specific to people with diagnosis of Neurodevelopmental Disorder (ASD, ADHD and IDD).
To support work to develop and use appropriate screening tools for Neurodevelopmental Disorder as part of an integrated care pathway
To develop knowledge and skills in treatment of patients with Neurodevelopmental Disorders (ASD, ADHD and IDD)
To deliver a range of adapted psychosocial, and psychoeducation sessions for individuals with neurodevelopmental disorders and helps other adapt similar programmes.
To develop networks and contacts to provide additional advice and research as resources for colleagues to support professional and service development.
To demonstrate a working knowledge of the key elements of relevant Mental Health legislation.
To take responsibility for a caseload of clients, maintaining clinical skills.
To demonstrate a positive attitude to mental health and neurodiversity, attending to patients with respect and dignity and have the ability to challenge staff where the standard falls below expectations.
To ensure that professional boundaries are always adhered to.
To keep up to date with issues and developments in nursing, the prison service and the wider NHS.
To keep abreast of new Policies, Guidance and Best Practice guidelines.
To provide advice and guidance to junior staff and those with no ND experience. Ensuring on going assessment, planning, implementation and evaluation of patient care, is in line with current best practice
To actively participate in the Healthcare Care Quality Meetings and other appropriate meetings as agreed.
To maintain up to date accurate clinical records both written and electronic.
Maintain accurate, timely paper and electronic records. Ensure they are kept safely in accordance with CNWL policies, including the Data Protection Act and the CNWL Confidentiality Policy.
To ensure that key local and national policies are implemented at a local level giving support to team members where necessary.
To lead on specific care quality initiatives, within the team and service wide.
To become involved in forums other than health to promote closer working practices in order to meet patient and carer needs.
To participate in the planning and implementation of clinical audits in the area and ensure that standards of good practice are met by all staff.
MANAGEMENT OF RESOURCES .
STANDARD
Work within agreed levels of resources, both human and financial, to support the clinical environment. To efficiently co-ordinate all services delivered to the patient groups within prison.
INDICATOR
To be a leading member of the multi-disciplinary team, contributing in all its' forums, attending and running meetings and case conferences for patients.
Leading by example, motivating and empowering others and promoting positive attitudes, mutual understanding and collaboration between all services involved in the care process.
To ensure the provision of a stimulating learning environment for other staff and students within the team.
To participate in managerial supervision and the appraisal process.
To identify skills currently held by all staff and co-ordinate appropriate training necessary to improve and enhance patient care, in accordance with trust policy.
To contribute to the delivery of Neurodevelopmental Disorder training for all prison staff.
