As part of our commitment to reducing the number of people who experience a long stay in hospital, we are recruiting a Social Care Coordinator to work within in the Selby locality area. If you are an experienced worker looking for a new challenge, we hope you will want to join us.

Reporting to North Yorkshire Council, Health and Adult Services, Intermediate Care Managers and Collaborative Care Team you will be part of a small, supportive dedicated team working in Urgent Care. You will work closely with our social workers, social care coordinators, therapists, nurses and healthcare support workers towards preventing admission to hospital and facilitating early discharge to a range of care settings.
