About the Team

The Enhanced Pathway provides evidence-based and therapeutic interventions with people with complex mental health needs. Our teams work collaboratively with service users and their families and carers, to enable them to live well in their communities.

We are looking for individuals who are flexible and creative in response to service user and organisational needs and who are committed to working collaboratively with other disciplines, to develop high-quality, strengths-based and recovery-focused packages of care and support.

The team is a multi-disciplinary team spanning Kirklees Council and the NHS Trust. It includes, social workers, CASO's, community psychiatric nurses, occupational therapists, psychologists and psychiatrists. We are passionate about offering our services users the best interventions to develop their skills, support their independence and their autonomy.

The Role

As a Senior Community Assessment Officer, you will play a key role in delivering statutory social work support. You will manage a caseload of patients, and lead on Care Act assessments, Mental Capacity assessments, safeguarding, and support planning, ensuring a seamless experience for patients and carers. You will be expected to be a lead for the Community Assessment and Support officers providing support, advice and guidance to develop their skills and confidence. You will play an important part of the Multi-disciplinary team in ensuring social care remains at the heart of discussions about are and support.

Key Responsibilities Include:

Clinical Duties:

  • Conduct home visits and complete assessments under the Care Act and within the scope of the mental health team.
  • Manage caseloads through assessment, support planning and discharge.
  • Provide support and crisis intervention as a supportive measure to the wider MDT.
  • Maintain accurate records on MOSAIC and SystmOne.
  • Participate in MDTs and make onward referrals to relevant services.
  • Support complex cases and respond to changes in patient risk.
  • Undertake a range of duties including triaging referrals and liaising with families.

Social Work Duties:

  • Provide advice on social care issues including carers assessments and financial support.
  • Complete Care Act assessments and develop care/support plans.
  • Conduct mental capacity assessments and carers assessments.
  • Attend weekly MDTs and complex case discussions.
  • Offer safeguarding advice and support to colleagues.

  • A Senior CASO or experienced CASO looking to further develop their skills and knowledge.
  • Strong assessment and care planning skills.
  • Excellent communication and multi-agency working abilities.
  • A compassionate, person-centred approach to care.
