About The Role
As a Specialist Navigator, you will offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following a discharge from hospital. Providing intensive one to one support to individuals to access statutory assessment and supporting the development of a holistic and person centred, strength-based personalised plan according to the needs of the individual.

You will work as a single point of contact connecting the Nottinghamshire Healthcare Trust, Nottingham City Council's Housing Solutions service and accommodation providers to assist with the process of acquiring and then resettlement into accommodation following discharge from hospital.

About You
The ideal candidate will have:

  • Knowledge of local social and healthcare services and an ability to navigate systems and pathways into services
  • Experience of supporting individuals with complex needs (e.g. rough sleeping, substance misuse, physical and mental health needs)
  • Understanding of the wider issues impacting upon homelessness, the barriers facing people with multiple and complex needs and the challenges involved in resolving rough sleeping
  • Evidence of working successfully in partnership across different sectors, building and maintaining good working relationships
  • Ability to advocate and negotiate on behalf of citizens and to give independent advice
  • Knowledge of housing legislation, rights and responsibilities in both the public and private sectors

A DBS enhanced check is required for this post for a regulated activity.
