The ideal candidate will have a strong background in senior care, disability support, or home health services, with a commitment to maintaining client dignity and independence.

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Provide medication reminders and assist with medication administration following prescribed care plans
  • Monitor and record vital signs such as blood pressure, pulse, and temperature
  • Support clients with meal preparation, feeding, and nutritional needs
  • Perform light housekeeping tasks such as cleaning and laundry to maintain a safe and comfortable environment
  • Assist with transfers using equipment like Hoyer lifts or other mobility aids
  • Observe and report any changes in client health status or behavior to supervising healthcare professionals
  • Support clients with cognitive impairments including dementia or Alzheimer's care, ensuring safety and engagement
  • Follow HIPAA guidelines to protect client confidentiality at all times
  • Implement individualized care plans tailored to each client's specific needs

Skills

  • Prior experience in home & community care, assisted living, nursing home, or long-term care preferred
  • Knowledge of patient observation, patient monitoring, and vital signs measurement
  • Certifications such as First Aid and CPR are highly desirable
  • Experience working with people with developmental disabilities, autism, or dementia enhances effectiveness in this role
  • Ability to perform heavy lifting safely when necessary for transfers or mobility assistance
  • Strong communication skills for effective interaction with clients and healthcare teams
  • Compassionate attitude with patience and empathy for diverse client needs
  • Familiarity with HIPAA regulations to ensure privacy and confidentiality in all interactions
  • Ability to work independently while following established care plans and protocols
