The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. Community Support Workers work with our multidisciplinary integrated team in Penwith (Penzance, Hayle , Marazion and St Ives and the surrounding areas) delivering reablement as part of a collaboration between community, acute and social care providers.
The reablement approach supports people to return to or remain in their homes. We are looking for candidates with the ability to motivate individuals in order to aid their recovery and maximise their independence, achieve their goals and get home from hospital quicker
You will be part of a dedicated team and will be provided with bespoke training to support your development in this role. You will need to demonstrate good communication skills and a positive approach to older people who may have communication difficulties or who may have challenging and complex health needs including frailty.
The team provide a 7 day a week service to community patients.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
Main duties of the jobThe main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting
The reablement workers within the locality form a part of an Integrated Care Pathway for older patients that involves a range of services to prevent hospital admission and support discharge from hospital
The post holder will be expected to deliver packages of care to patients in their own home; this may also include residential homes and nursing homes.
The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service.
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.

Essential criteria
- NVQ Level 3 qualification in care or equivalent experience
- Evidence of experience in care setting
- GCSE Maths & English or equivalent qualification
- Additional relevant training in Healthcare
