Proclaim Care has an excellent opportunity for experienced Rehabilitation Case Managers to work with us on an associate basis. We are seeking associates based throughout the UK.
Summary of Essential Requirements:
- Right to work in and current resident of the UK
- Qualification in Occupational Therapy, Nursing or Physiotherapy
- Active registration with the HCPC or NMC
- Valid driving licence
- Willingness and ability to travel within the UK
- Experience of working as a Rehabilitation Case Manager in the personal injury/medical negligence sector

Job Description:
This is an exciting opportunity to support service users with complex injuries and health conditions to optimise their recovery and reach their rehabilitation and return to work goals. Working within a commercial (personal injury and medical negligence) context, this field-based role involves navigating service users through their personal rehabilitation journey by:
- Conducting in-person and virtual assessments to determine service users' physical, psychological and social needs on an ongoing basis
- Devising and coordinating goal-focussed rehabilitation programmes, which includes a combination of both private and statutory services
- Collaborating with family members and other inter-disciplinary professionals
- Communicating regularly with external stakeholders e.g. solicitors, insurers and employers
- Writing comprehensive reports for the instructing parties to provide updates, demonstrate progress and seek funding approval
- Facilitating return to work and/or education
Person Specification:
We are looking for experienced Rehabilitation Case Managers with a qualification in Occupational Therapy, Nursing or Physiotherapy and active HCPC or NMC registration. A proven clinical background in complex orthopaedic trauma is required.
Experience of, or interest in, working with the following clinical specialisms would be of benefit, but is not essential:
- Acquired brain injury
- Spinal cord injury
- Amputations
- Paediatrics
Rehabilitation Case Managers work from home but regional travel to meet with service users, treatment providers and employers is a large part of the role. A valid driving licence is therefore essential.
To excel in this role, you should demonstrate:
- Strong clinical reasoning skills with a solutions-focussed approach
- Effective communication, report writing style and interpersonal skills
- Excellent organisational and administrative skills with attention to detail
- Confidence with utilising IT software packages
- The ability to work independently and as part of a multidisciplinary team
