Job Purpose
The Senior Care Coordinator is responsible for ensuring the safe, effective and efficient coordination of home care services for service users across Peterborough. They support the Registered Manager in delivering high-quality, person-centred care in line with CQC Standards and K2 Care's values.
This senior office role combines rota management, staff supervision, quality assurance and relationship management to ensure smooth day-to-day operations across the service.
Key Responsibilities
1. Care Coordination & Service Delivery
- Manage daily scheduling and rota planning to ensure all care visits are covered safely, on time and in line with individual care plans.
- Allocate Care Workers based on skills, compatibility, continuity and geographical areas.
- Respond promptly to last-minute changes, staff absences and emergency situations.
- Support the creation, review and updating of person-centred care plans and risk assessments.
- Maintain strong communication with service users and their families regarding changes and ongoing needs.
- Ensure accurate recording of all changes, updates and care adjustments on care management systems.
2. Staff Support, Supervision & Leadership
- Provide day-to-day support and guidance to Care Coordinators and Care Workers.
- Assist with inductions, shadowing, spot checks, competency checks and ongoing staff mentoring.
- Participate in staff supervisions, appraisals and performance reviews.
- Identify staff training needs and support development pathways.
- Participate in the on-call rota, including evenings and weekends as required.
3. Quality, Compliance & Safeguarding
- Ensure all care delivery meets CQC Fundamental Standards and K2 Care's policies.
- Quality-check daily records, MAR charts, visit logs and electronic care notes for accuracy and compliance.
- Escalate concerns regarding service delivery, safeguarding, staff conduct or service user wellbeing to the Registered Manager.
- Support with incident reporting, complaint handling and professional follow-up.
- Contribute to audits, service improvement plans and internal quality monitoring.
4. Communication & Relationship Management
- Act as a senior point of contact for service users, families, staff and external professionals.
- Build and maintain strong working relationships with social workers, district nurses, GPs and partners.
- Engage in multidisciplinary meetings, reviews and professional visits.
- Promote K2 Care's values, professionalism and high service standards at all times.
5. Administration & Systems
- Maintain accurate and up-to-date records on the care management and rostering systems.
- Ensure documentation meets regulatory, legal and organisational standards.
- Produce data reports, compliance information and service delivery statistics when required.
- Support updates to policies, procedures and internal documentation.
