About the role
We are looking for a compassionate and dedicated Outreach Support Worker to join our team. In this rewarding role, you will provide practical and emotional support to vulnerable adults living in the community, helping them to maintain independence, improve their well-being, and achieve their personal goals.
You will work with individuals facing a range of challenges, including mental health issues, homelessness, substance misuse, and social isolation. The role involves visiting service users in their homes or in the community, offering one-to-one support and advocacy to help them access services and live fulfilling lives.
Key Responsibilities
- Provide person-centred support tailored to each individual's needs, goals, and preferences.
- Assist service users in developing life skills such as budgeting, cooking, personal care, and maintaining a tenancy.
- Offer emotional support, encouragement, and guidance to help individuals build confidence and resilience.
- Support clients in accessing health, housing, education, and employment services.
- Work collaboratively with external agencies, families, and professionals to ensure coordinated care.
- Maintain accurate and up-to-date records, risk assessments, and support plans.
- Promote independence, dignity, and choice in all aspects of support.
- Participate in regular team meetings, supervision, and ongoing training.
About You
- Previous experience in a support or outreach role (mental health, housing, or social care setting preferred).
- A caring, patient, and empathetic approach with strong communication skills.
- Ability to work independently, manage a caseload, and travel between visits.
- Understanding of safeguarding, confidentiality, and professional boundaries.
- Basic IT and record-keeping skills.
- A valid UK driving licence and access to a car (desirable but not essential).
