Healthcare Support Worker

To support health promotion and prevention activities, including smoking cessation, weight management and lifestyle interventions. Supporting patients with long-term conditions and health inequalities. To assist and support medical personnel int the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.
To act as a focal point of communication between patients, doctors, nurses and other members of the practice team. To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the nurse manager so these can be addressed. General The HCSW will be required to act in a professional manner at all times and adhere to practice policies, protocols and guidelines. Full training will be given.
Training requirements will be monitored by yearly appraisal. Personal development will be encouraged and supported by the practice. Safeguarding Safeguarding is everyone's responsibility and all employees are require to act in such a way that at all times safeguards the health and well-being of children and vulnerable adults. Familiarisation with, adherence to, the appropriate organisational Safeguarding Policies and any associated guidance is and essential requirement of all employees as it participation in related mandatory/statutory training.
All employees must ensure that they understand and act in accordance with this clause. Confidentiality In the course of seeking treatment , patients entrust us with or allow us to gather sensitive information relating to their health and other matters . They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in the Job Description, the post-holder may have access to the confidential information relating to the Practice as a business organisation.
All such information from any source is to regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to the confidentiality and the protection of personal and sensitive data. Health & Safety The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy and the practice Infection Control Policy. Equality & Diversity The post-holder will support the equality, diversity and rights of patients, carers and colleague.
Personal/Professional Development The post-holder will participate in any training programme implemented by the Practice as part of this employment, such as training to include: Participation in and annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work .
